CHAP Membership Cards

CHAP Self-Printable Membership Cards: Use this form to print out copies of your Membership card for CHAP discounts and identification. Log in below, with your email address and password, and we'll provide you with a downloadable PDF file you can print out on heavy card stock. Cards will be provided for each spouse in the case of married couples. They contain your name, your Member ID, and your membership expiration date.

If you do not have a password, simply input your email address and leave the password blank; the system will generate a password for you and mail it to you.
